HOLD HARMLESS AGREEMENT

I/we agree to release the Mattie Williams Neighborhood Family Center and Kiwanis Club of Safety Harbor Foundation, Inc. from any liability of any kind, for any and all damages arising out of any loss or injury resulting from my participating in any activities of any kind in any way connected to the Mattie Williams Neighborhood Family Center and Kiwanis Club of Safety Harbor Foundation, Inc., or use of any facilities or equipment made available by the Mattie Williams Neighborhood Family Center and Kiwanis Club of Safety Harbor Foundation, Inc. This release includes a release for any and all losses or injury arising out of any act or omission or negligence of the Mattie Williams Neighborhood Family Center and Kiwanis Club of Safety Harbor Foundation, Inc., its agents, employees, or activity supervisors specifically concerning or arising out of Mattie Williams Neighborhood Family Center and Kiwanis Club of Safety Harbor Foundation, Inc., activities.

This release constitutes a release or waiver of all claims against the Mattie Williams Neighborhood Family Center and Kiwanis Club of Safety Harbor Foundation, Inc., including those claims arising out of the negligence of the

Mattie Williams Neighborhood Family Center and Kiwanis Club of Safety Harbor Foundation, Inc., its agents, employees or supervisors. This release and waiver is limited to activities conducted by or on behalf of the Mattie Williams Neighborhood Family Center and Kiwanis Club of Safety Harbor Foundation, Inc.

Please read carefully before signing. This release is signed of my/our free act and will. I/we am/are of legal age. By signing this document either in paper form or electronic form, I/we agree to abide by all rules and regulations set forth by the Bands on the Bay committee. I/we understand that the Committee may change or modify the rules as needed and any failure to abide by this Hold Harmless Agreement or the rules and regulations set forth by the Committee may result in revocation of my/our approval to participate in this and other events held or sponsored by the Mattie Williams Neighborhood Family Center and/or Kiwanis Club of Safety Harbor Foundation, Inc.
